Student Application Form

All new students will need to fill out an application form for our records. Returning students do not need to fill out a new application.

Students (18+) should fill out the form with their information and answers.

Parents/Guardians of minors should fill out the top portion (contact information, emergency contacts). The Student Questionnaire portion should be filled out by the student though a parent or guardian can help where necessary.

Gift Purchases - Please let us know you are purchasing a class as a gift. During checkout, there will be an area to fill out the student's name so their spot will be held for the class. If the gift is a surprise, the Application Form can be turned in at a later date, but before the first day of class.

PLEASE FILL OUT THE FORM, SAVE IT TO YOUR COMPUTER/PHONE AS [Student's Name] Application Form, AND EMAIL IT TO US AT
info@creativeforgeacademy.com WITHIN A COUPLE OF DAYS AFTER ENROLLING IN THE COURSE.
